Add support for TTML (EBU-TT-D) subtitles

Updated patch that addresses the comments made in the above review and also
contains fixes for a couple of issues:
  - Fixes issue where first word in line of live-generated "snake" subtitles
shifts vertically a little when subsequent words appear.
  - Fixes issue where the rightmost character in italic text is clipped
slightly.
