Patch implementing STAP-A for the RTP H.264 payloader

Thanks for the quick review!

Here's an updated version of the patch, with the debug code removed and
"do-aggregate" enabled by default.

It's a bit hard to estimate how risky it is to enable by default; I haven't
seen any problems, but who knows what's out there...
