[gst-devel] Updated LICENSE_readme in gst-plugins

At David Schleef's request, I updated the LICENSE_readme file
in gst-plugins.  I removed old plugins, added new ones, and
updated plugins so the correct name is now used.  I added URL
information for most of the plugins.

Not sure if people will like the disclaimer I added to the top,
and some plugins are missing URL's and library names.  So perhaps
people can let me know the missing information and I'll update it,
or people can update the file directly with the correct info.

Though, please review, I may have gotten some of the information
wrong.  Some of the plugins have pretty generic names so I may
have found the wrong website for some plugins.  And some of the
plugins I listed in the "other free license" section may really
be BSD licenses, but I'm not sure.
