HAL 0.5.7 -> 0.5.8 API changes (and subtle breakage heh)

I don't think HAL is internal-API stable. I think externally (methods
and LibHal and things) we are pretty good at preserving compatibility,
but internally I think it's fair game to change structures. I do think
the consequences of changing of the index->idx name was perhaps not
fully understood, and if you see any other API changes like this in the
future, then please SHOUT LIKE MAD, and we'll see what we can do. :-)
