Couple of notes:

 1) It appears to be an issue with texture coordinates getting messed up.  Not
sure exactly how yet but that seems to be the actual problem.
 2) The bug goes away if INTEL_SCALAR_VS is set to 0 so it's some sort of
miscompiled vertex shader.
