Description:
1. Open or create a LibreOffice Base Database
2. Menu Bar → Insert → Form…
3. Show last item of Insert menu “Form Control >” at:
Form Menu Bar → Insert → Form Control >

Likewise for:
1. Open or create a LibreOffice Base Database
2. Menu Bar → Insert → Report…
4. Show last item of Insert menu “Form Control >” at:
Form Menu Bar → Insert → Form Control >

This would provide consistency across LibreOffice as Writer, Calc, Impress have
“Form Control >” as the last item in the Insert menu list.
