Have problems to understand the bug-description.
1. Open a *.odb-file
2. Menu Bar → Insert
... there I could read
Form
Report
...

There is no item
"Form Control >"
because there is no possibility to create a form control without a form.

If you open a form for editing, not input data, the toolbar "form-controls"
should be shown automatically. Do you want to get form-control in an opened
form through
Insert → Form Control → ...
?
