Thank you Robert for looking into this bug report.

Continuing from where you got stuck, click on Form...
then the Database Form window opens.
Then in the Database Form window, select: Menu Bar → Insert
There is no "Form Control >" as the last item like the rest of the LibreOffice
suite. The bug report it to recommend adding it.

Attached is a mockup with "Form Control >"
