So the SP1 Platform Update (KB2670838) is probably installed.

And the troublesome Toshiba mobile deployed 8.15.010.1883 Intel GPU driver,
which others also reported as bad. This particular GPU does work correctly with
current driver.  -> FIXED

For @tomangelo,

You show in comment 0 that your working with a system with Intel i5-5200U (that
has an HD5500 GPU embedded) and an AMD Radeon M375 discrete GPU. Is this laptop
a Lenovo z51-70?

Also in comment 10 that you'd tried the 17.4.4, 17.4.2 and 16.12.1 drivers.

But could you post your DxDiag.exe result for System & Display, so we can see
what vendor, GPU, and driver you are working with.

Also, current AMD drivers for the R9 M375 are at 17.7.2--worth checking that.
