I removed my user profile and I can reproduce this. Windows running 6.1.5.2.

To reproduce
1. Create new Impress document
2. Save as Template with name Default in My Templates. (File...Templates...Save
as Template... Type "Default" in Template Name and select My Templates.
3. Look in templates folder. On my install it's located in
C:\Users\<user>\AppData\Roaming\LibreOffice\4\user\template. You should see a
file Default.otp
4. Create new Draw document
5. Save as Template with name Default in My Templates. (File...Templates...Save
as Template... Type "Default" in Template Name and select My Templates. You may
be asked if you want to overwrite Deafault template. This is the first clue
that something is broken. Click yes.
6. Look in templates folder. "Default.otp" has disappeared. It has been
replaced by "Default.otg"
