Data Provider GUI 6.4.2.2

The Start and Column menus are absent from the menu-bar of the Data Provider
dialog. Possible reversion to
https://bugs.documentfoundation.org/show_bug.cgi?id=122256 in 6.2.0.1; was
present in 6.3.5.2
