in master / Writer the menu File > Templates > Manage opens the Form design
toolbar
Not sure if it is a bug in this reorganization or if the root cause is
elsewhere. Indeed in my build of LO 5.0 beta3, the same menu opens the media
playback toolbar in a French UI and the Form Design toolbar in the EN UI).
