Manual CDMA activation

Hey,

The 'aleksander/manual-cdma-activation' branch in upstream git contains
the logic to implement the ActivateManual() method from the CDMA
interface, currently only for QMI-based modems. This method allows
setting 'spc', 'sid', 'mdn', 'min'... in the CDMA modem.

The branch seems to work well, but I wouldn't mind another review before
merging to git master. You'll need libqmi from git master if you're
going to test it.
