[ooo-build] kde4 patches

Update patch for KDE4 integration.

Patch created with git format-patch from the current master branch.

New things:
- implementation files split off instead of one giant file
- cleaned up the include files and removed the catch all kde4_headers.h include
- changed header guards for all related kde4 header files to use
'#pragma once' instead of defines
- file picker now works (still has a glitch with the checkbox labels)
but opens and saves :)
