Try to use exactly the xorg.conf sample from 
http://wiki.openchrome.org/tikiwiki/tiki-index.php?page=TVOut
Especially, narrow the VertRefresh and HorizSync.
Also the conf you include says 720x576, whereas the log set 720x576Over. 
Try alternatively with both of them.
Also, I thing VBEModes will to more harm than good here so make sure to 
not use it.
The BIOS output mode should not matter, but just to be sure, set it to 
TV only.
Also don't connect a VGA monitor, it might be detected and confuse the 
driver (or at least trick it into not using the tv encoder mode).
