I generated most of them using iText and Acrobat 7 on Mac. Some were donated 
by Leonard Rosenthol (and are very much appreciated). I think at least one 
was generated using pdftk.
They normally have weird combinations (eg each page has a different 
orientation value) so I could test meta-data.
