[poppler] [PATCH] Improved Forms Support

I've updated my earlier patches that rewrote some of the handling of 
form fields in poppler.  The updates include moving the new functions I 
introduced from Annot to AnnotWidget.

The new patches, against git commit e0eff92c7067, are attached.  I did 
the rebase fairly quickly and haven't completely tested yet, but it does 
compile and seem to work on a few test documents.

Let me know if you see any problems.
