This is kinda irritating. Basically, whenever I play *any* video, *any* sound
clip, hell, any *anything* on Iceweasel, PA basically causes it to fire at
maximum volume no matter what my other settings say. This also happens if I
skip back or forward in the video or audio in question.

I have not seen this behaviour anywhere else on my system, or with any other
audio or video programs.
