browser media frameworks are known to do that (to explicitly set volume to
100%), yes (I know webkit-based ones do at least, apparently mozilla too). 
They arguably shouldn't do that (most other apps that used to do that have been
fixed, as you noted "i have not seen this...").

Only workaround, that I'm aware of at the moment, is to disable pa flat volumes
