Since I had a similar issue, make sure that DISPLAY is unset when you
ssh on user_b at localhost.

The variable is passed to the PA clients (at least), and they can't
connect to the system instance and try to connect on user_a at localhost
somehow, and fail  (as it seems to me).

PA should probably be more verbose about this issue, I probably had it
in the past, but couldn't find the reason.
