I am rather positive to all the changes you are proposing. They are
small, and don't affect much the size of the window.  But HIG wize, the
Send button will have to be bigger (see Button default size).

The Add Smiley button is something my grand-ma requires to use them.
Again good.

As for avatars, if you use one of the nice Adium themes, you don't need
to have theme in the chat window UI it self.  For example, Rankoo has
them :

May be we could introduce a Toolbar in that UI that would contain: 
      * Get info (opens the Information dialog)
      * Insert smiley
      * History
      * Start audio/video chat
      * Send file
