what creates fonts.alias?

I installed fonts-misc-misc which uses bdftopcf, ucs2any (from font-util) 
mkfontscale, and mkfontdir.

Starting Xnest (or Xorg) complains:

  could not open default font 'fixed'

I manually created the fonts.alias for this and that worked.

The mkfontdir documents the fonts.alias file. But it doesn't say what 
creates the fonts.alias.

Where does it come from?

There was a mkfontalias at http://home.c2i.net/dark/linux.html#fuzzy for
creating a fonts.alias from the fonts.dir for TrueType fonts. (But now 
gone.)

I am sure something is with Xorg, but I am overlooking it.
