is the problem a unique author who doesn't understand the PD license ?
In this case, it's easier delete his contributions.

if ocurrs a second or a third case,  we can sent a email to all authors (we
have their email?) explaining the implications of PD license, and also,
change the form...

the 'new form' is good, but i prefer a individual solution for a individual
problem.
