There are rows in cc_tbl_uploads that refer to physical files that
were manually deleted from your content directory? Is that right?

The safest way to do it, to maintain the integrity of the meta data,
is to write a custom one-time script that checks the existence of
every single file and then calls
CCUploadAPI::DeleteUpload($upload_id).
