If you NEVER use the gs operator - then the fact that you
have an ExtGState is irrelevant. It is NOT used.
What you may be seeing, assuming you are using Acrobat
Reader to view, is that Acrobat uses different techniques to render
100% opaque pages vs. those with some transparency. So things like
anti-aliasing and such will change and may appear different to
you. But it isn'.

Yes, you do.
You don't have to use it when you do NOT use transparency -
the defaults are fine. But if you WANT transparency, you need to use
it along with a gs.
