I've since been able to figure out the problem with help from #pulseaudio.

The main "fix" was switching the streams.

Also, it may or may not have been necessary to uncheck "allow skype to
automatically adjust my mixer levels" in Skype. My attempts pre
#pulseaudio had this box checked.
