I've attached a diff with the above-promised modifications.

1) I completed it late last night so I'd appreciate the extra eyes 
looking at it, they probably need (if nothing else) a copyright blurb 
or something similar

2) I'm not an XSLT wiz (in fact this was my first exposure to it) so 
feel free to modify and poke holes at it

3) I doubt you end up with perfectly conformant HTML; that's not one of 
my goals

It appears to work well on my setup so I hope I haven't forgotten 
anything obvious.
